1095-A - If you bought insurance through the Marketplace, this form will be mailed to you by the end of January by the Marketplace (healthcare.gov). This is your proof of having insurance coverage in 2014. You may get more than one form if you had more than one policy or if not all family members were on the same policy. If you don't get one by February 2, please call the Marketplace at 800-318-2596 to request another copy. This form shows how much your insurance cost and how much of a tax subsidy you received each month. You will not get this form if covered under group or individual plans purchased outside of the Marketplace, MI Child, Medicare, or CHIP.
Form 8962 - This form will now be part of your tax return. The purpose of this form is to adjust your subsidy based on your actual income. When you applied for coverage for 2014, you estimated your income. Now it's time for the subsidy to be adjusted according to your actual income. If your income was less, your subsidy will be increased. If your income was more, your subsidy will be decreased. This difference will be included in your 1040 and will affect your refund or tax due. A repayment plan will be required if you owe taxes and can't pay in full. The IRS has waived additional penalties for those who received too much in subsidy as long as the rest of the taxes due are current.
