Review and choose the plan best for your family

• Fill out the application completely
• Sign the application
• Include your check or credit card authorization for United HealthOne 
• Return the application and payment to fax 888-715-1289 or mail to:

We will contact you upon receipt of your application and to let you know when it has been approved.

You will receive a card and handbook in the mail within a few weeks.
